Manager, Habilitation Services (Day Center) – Fairhaven, MA

Founded in 1985, Community Connections, Inc. offers an extensive array of services combining traditional and alternative therapies, along with community activities and life skills training, to promote a healthy and productive lifestyle for adults with developmental disabilities.   Our Day Centers provide habilitation services that are designed to meet individual needs, and help people achieve their goals by providing high quality care that builds on the strengths of the whole person. 

 

Community Connections, Inc. has been accredited at the highest level from the Commission on Accreditation and Rehabilitation Facilities (CARF) since 1987; we currently serve over 675 clients at nine locations throughout Southeastern Massachusetts.  We offer six service programs staffed by 282 dedicated employees.

Position

Manager, Habilitation Services:  The Manager is responsible for the daily and ongoing operation of all facets of the habilitation program under his or her authority, directly supervising all Habilitation Service staff.  This role reports directly to the Service Director. 

Responsibilities

The Manager is responsible for overseeing the daily operations of the Day Center, to include ensuring quality of services delivered; coordinating all referrals and serving as a liaison with funding sources; supervision of all staff; development and implementation of service design; and achievement of Habilitation Services outcomes and margin goals. 

Qualifications

Proven leadership skills, with at least 5 years of managerial experience required, preferably in a Habilitation/Human Services setting.  Bachelor’s degree required.  General knowledge of related clinical services and Medicaid regulations preferred.  Salary commensurate with experience.

Director of Adult Day Health Programs

 

MI Health/Care Services is a nonprofit, Catholic-oriented, mission-driven facility.

Our ADH programs offer Mon-Friday, 9 to 5 Nursing and Social Support at two locations in the community.  Our programs have received regional and national recognition as well as the prestigious National Achievement Citation from the Catholic Health Association.

The Director of ADH is charged with directing the overall operations and growth of MI's ADH programs in accordance with current federal, state and local regulations that govern ADH programs to assure the highest degree of quality care can be provided to our clients at all times.  This includes our bilingual/bicultural program serving older adults who primarily or exclusively speak Spanish.

Responsibilities of the position include, but not limited to: promote and support the growth and development of high quality ADH programs; maintain regulatory compliance; coordinate the marketing and admissions process; manage budgets for both sites; maintain positive relationships with families and clients; conduct community awareness programs to keep the community informed of the organizations services; supervision of staff.

 

Qualifications:

Requires BS degree in Nursing from an accredited college with current MA RN license; three plus years experience in the healthcare community; knowledge of community healthcare resources within the Merrimack Valley.  Bi-lingual in Spanish a plus.
