2007-08 Membership Categories & Dues: ASSOCIATE
Consultants, Vendors and Other Organizations Not Providing Adult Day Services
MADSA works on behalf of its provider members to promote access to quality daytime care for adults with long-term care needs. Associate members are non-provider companies, individuals or organizations which offer goods, services or support to the adult day services industry. MADSA offers Associate membership in the following categories: Individual Consultant, Vendor, and Associations/advocacy groups.
ASSOCIATE (NON-VOTING) ANNUAL DUES: BASED ON ASSOCIATE CLASSIFICATION
INDIVIDUAL/CONSULTANT...................................................................................................................................$150 ANNUAL DUES
Individual or Business offering start-up or operational consulting services to adult day services providers. Consultants who hold any of the following relationships with one or more adult day services facilities, must bring each such facility into membership before becoming eligible for Associate-Consultant membership: (1) any Stockholder or operating entity(2) Administrator of Record, (3) Program Director of Record, (4) Employee.
VENDOR................................................................................................................................................................$450 ANNUAL DUES
Business offering products / services to the adult day services industry. Vendors who hold any of the following relationships with one or more adult day services facilities, must bring each such facility into membership before becoming eligible for Associate-Vendor membership: (1) any Stockholder or operating entity, (2) Administrator of Record, (3) Program Director of Record, (4) Employee. Those providing start-up or operational consulting services are ineligible for Associate-Vendor membership.
ASSOCIATIONS/ADVOCACY GROUPS. ................................................................... $ 150 ANNUAL DUES Community Based long-term care organization, association, or network not directly providing adult day services. Groups advocating health care of individuals and caregivers. Examples: Area Agency on Aging, Caregiver Resource Center, Regional Center, Network association, educational institution, etc.

MADSA strives to encourage quality, efficiency, leadership and innovation by providing timely information, technical assistance, education, and business technology. MADSA is a non-profit 501 (c) 6 mutual benefit organization.

Application Process
To apply for membership, fill out a membership application and return it to the MADSA office. Application for membership will be processed when the application and related dues payment (see attached) are received. All incoming memberships are considered Provisional Membership, pending review and approval by the MADSA Board of Directors at its monthly meeting. After the application is approved, you will be mailed a Certificate of Membership. In the event the application is declined by the Board of Directors, notification will be made by mail and a refund is issued.
