Vendor Table Application The Massachusetts Adult Day Services Association (MADSA) is a well-established membership organization with over a 25-year history of outstanding representation of adult day programs all across the Commonwealth. MADSA currently represents 115 Adult Day Programs in the state. MADSA’s mission is to promote and support the growth and development of high quality Adult Day Services. This is done by advocating for adequate reimbursement and regulatory and administrative relief, and through the provision of information, education, and opportunities for networking and sharing best practices. MADSA is affiliated nationally with the National Adult Day Service Association (NADSA). We are currently seeking applications for Vendors interested in a display table at our annual and quarterly meetings. If you are interested in securing a table please complete the information below and fax the application to Kathy Kilhart at (978) 632-4513 or via email to kathy.kilhart@gardnervna.org. Checks are made out to MADSA and mailed attention to Kathy Kilhart, Gardner VNA, 34 Pearly Lane, Gardner, MA 01440. The cost per table is: $ 75.00 for members, $100 for non-members. Directions to College of the Holy Cross/Hogan Campus CenterBy Car:From the East and West (Boston, Hartford and New York)Massachusetts Turnpike I-90. Take Exit 10 to I-290 East toward Worcester. Take Exit 11 (College Square/Southbridge Street) off I-290. Cross over to the right lane immediately after coming off the ramp to College Square. Take the first right (before the traffic light) onto College Street. Go up the hill and enter the last gate on the left, Gate 7. The Hogan Campus Center is the second building on the left with the large silver cross on it. Visitor parking is to the right of the Campus Center. MapQuest may provide door-to-door driving instructions. From the NorthI-495 South to I-290 West, Exit 25B. From I-290 West take Exit 11 (College Square, Southbridge Street). Bear left coming off the ramp onto Southbridge Street. Take the first right (before the traffic light) onto College Street. Go up the hill and enter the last gate on the left, Gate 7.
